What is supervisory billing?
It is an insurer's rule that lets a licensed supervisor, or the supervisor's group, bill for covered sessions provided by an associate who is still working toward independent licensure. The associate is not credentialed with the insurer; the supervisor is.
- It is set by each commercial insurer in its own policy, so the conditions differ from one insurer to the next. It is not the same as Medicare's incident-to rule, which is federal and covers different practitioners.
- Most insurers list the supervisor as the rendering provider on the claim. Highmark works the other way in Pennsylvania and Delaware: associates are enrolled with Highmark and listed as the rendering provider on their own NPI, paid at 75% of the fee schedule. [4][5][8]
Which insurers allow supervisory billing?
Optum and UnitedHealthcare commercial plans, Aetna, Cigna (Evernorth), Independence Blue Cross, Horizon BCBSNJ, BCBS of Illinois and BCBS of Texas all publish rules that allow it. The conditions, the person on the claim and the modifier differ for each.
| Insurer | Allows it? | Who is on the claim | Modifier | Key limits |
|---|---|---|---|---|
| Optum and UnitedHealthcare, commercial (policy updated July 2026) | Yes in California, Colorado, Hawaii, Iowa and Massachusetts; in other states only if Optum previously approved it | Supervisor's name, credentials, NPI and tax ID; supervisor or group bills | U5 on each supervised line | Out of network only in those five states; no Medicare Advantage, evaluation and management visits or ABA |
| Aetna (Fall 2026 bulletin) | Yes, for license-eligible behavioral health clinicians | Licensed supervisor as rendering clinician | None stated | In network only; supervisor employed by the same group, not necessarily at the same location; virtual supervision allowed |
| Cigna (Evernorth, September 2026) | Yes, as state licensing rules allow | Fully credentialed provider as rendering provider | None stated | Supervisor must meet Evernorth credentialing standards |
| Independence Blue Cross (April 2026 manual) | Yes, for LSW or LMSW, LAMFT and LAPC | Credentialed, contracted supervisor bills | AJ | Commercial members only; not Medicare Advantage or when IBX is secondary; written member consent |
| Horizon BCBSNJ (policy revised November 24, 2025) | Yes, for LSW, LAC, AMFT and psychology permit holders with an approved supervision plan | Supervisor as rendering practitioner | HO | Supervisor in network; out of network not paid; not Braven Medicare Advantage; written member consent in the chart |
| BCBS of Illinois (notice of October 16, 2025) | Yes, for master's and doctoral interns and postdoctoral fellows | Supervising clinician's NPI | None stated | Supervisor available 100% of the time; at most four trainees each; same organization |
| BCBS of Texas (2025 billing FAQ) | Yes, with a formal supervisor relationship | Supervisor or clinic director as rendering provider | None needed | Supervisee under a trainee license; supervisor reachable to join the session |
| Highmark, Pennsylvania and Delaware commercial (July 2026) | Associates billed differently | LAPC, LAMFT or LSW on their own enrolled NPI | None stated | Paid at 75% of the fee schedule; claim rejected if not yet enrolled |
| BCBS of Michigan and BCN, commercial (June 2026) | Ending | Incident-to under the supervisor until February 28, 2027 | SA from September 1, 2026 | From March 1, 2027, providers with provisional licensure are no longer paid incident-to in a professional setting |
- Each row is taken from that insurer's own policy, manual or provider bulletin. Horizon, BCBS of Illinois and BCBS of Texas published theirs in 2025; they were still the live versions in October 2026. [1][4][5][6][7][8][9][10][11]
- Optum asks for the supervisor's details in boxes 17 and 17b of the claim form, with one supervisor per claim. That instruction comes from its billing reference guide, which is older than the July 2026 policy. [2]
Which insurers have no published supervisory billing policy?
We found none from Anthem, Humana, Magellan, Florida Blue, CareFirst, Premera or Regence. No published policy does not mean the insurer forbids it, so ask the insurer in writing before billing an associate's sessions.
- Anthem's only related document we found is its New York commercial incident-to policy for nurse practitioners and physician assistants, which pays 15% below the physician rate with the SA modifier. It does not cover therapy associates. [12]
- Carelon Behavioral Health, which manages behavioral health for Anthem in many markets, tells providers not to bill in their own name for services an intern, psychological assistant or other clinician provided. Its handbook also mentions a Supervised Clinician Group Roster form without saying which contracts use it. [13]
- Magellan requires each clinician in a group to be credentialed individually, and lists billing non-credentialed staff under a credentialed provider's name as an example of fraud. [14][15]
Does Medicare allow supervisory billing?
Not under that name. Traditional Medicare does not enroll associate counselors, but an associate working within state law can provide services incident-to a physician, physician assistant, nurse practitioner, clinical nurse specialist, certified nurse midwife or clinical psychologist.
- LCSWs, mental health counselors and marriage and family therapists cannot supervise anyone incident-to their own services under Medicare, according to Noridian, a Medicare contractor (article last updated March 28, 2025). [16]
- Behavioral health services billed incident-to need only general supervision: the supervising practitioner must be reachable but does not have to be on site. Only the supervising practitioner bills. [17]
- Optum's Medicare Advantage policy states that services by interns or postdoctoral graduate students cannot be billed incident-to. [3]
What about TRICARE and Medicaid?
We found no TRICARE route for pre-licensed associates. Medicaid rules are set by each state, so check your state's Medicaid agency and each Medicaid managed care plan you bill.
- TRICARE's "supervised mental health counselor" is not an associate category: the counselor must already be licensed or certified, and needs a physician referral and ongoing physician oversight. [18]
- Colorado is one example of a state setting its own rule: from July 1, 2026, Colorado Medicaid requires a U9 modifier on behavioral health claim lines for services by a pre-licensed clinician, and licensed social workers bill under the supervisor's NPI. [19]
- Optum's commercial policy says the same: for Medicaid and CHIP plans it follows the rules of the state where the services were provided. [1]
What should you check before billing an associate's sessions?
Check the insurer's current policy, then confirm the supervisor's network status, the claim format, the plans that are excluded and any consent rule before the associate sees that insurer's members.
Read the policy Find the insurer's current supervised billing policy for your state and keep a dated copy. If none is published, ask the insurer in writing.
Supervisor credentialed The supervisor, and the group where the insurer requires it, should be credentialed and in network with that insurer before any claim goes out.
Claim format Use the modifier that insurer names, or none if it names none, and list the person it asks for as the rendering provider.
Excluded plans Check whether the member's plan is excluded, such as Medicare Advantage, out-of-network benefits or cases where the insurer is secondary.
Consent Where the insurer requires it, tell the client about the supervisory relationship, share the supervisor's contact details and keep written consent in the chart.
Records Keep supervision notes as your state board requires. Optum says it may audit the records behind these claims.
