BCBA Credentialing for ABA Practices and Agencies
Medtransic handles BCBA credentialing for ABA practices and agencies: the agency's payer contracts and every BCBA who needs to be in network. Before we file, we check state licensure and the supervision rules each payer publishes. Every application and its stage is tracked in your client portal. Credentialing is $150 per application, per provider. In our experience it usually takes 60 to 90 days with each payer. There is no long-term contract.

What stalls ABA credentialing
Most delays come from the same few gaps. Each one is easier to fix before an application goes in.
Opening on BACB certification alone
BACB certification is not a state license. BACB's own table lists 40 states plus DC with behavior analyst licensure laws, and Colorado's law (2026) is the newest. Aetna requires every BCBA, BCaBA and paraprofessional to meet state requirements, so licensure has to be settled first wherever the state requires it.
A new BCBA starts before being credentialed
At Evernorth, the clinic contract comes first. After that, each certified or licensed autism provider must be credentialed individually. A BCBA hired after the contract is signed still needs an application of their own.
An RBT with no supervisor on record
BACB moves an RBT to inactive status when no supervisor is on record. An inactive RBT cannot practice, bill or present themself as an RBT. When a supervising BCBA leaves, the RBTs under them need a new supervisor on record.
Supervision that is not written down
Evernorth's ABA policy says the name and credentials of the person providing supervision must be documented. BACB asks supervisors to keep supervision records where they can be reached for up to 7 years in case of an audit.
Medicaid is checking credentials
In a February 2026 audit of Colorado Medicaid ABA payments, HHS OIG found 18 of 100 sampled enrollee-months involved providers who did not meet credential requirements. CMS's August 2026 ABA toolkit for states covers provider enrollment and credentialing.
Our side of the file, and yours
What we run
- A licensure check for each BCBA in each state where you see clients
- NPI checks for the agency and each BCBA, under the right provider type
- Agency contracts and individual BCBA applications with each payer
- Follow-up with every payer until approval, with the effective date recorded
- New applications when you hire a BCBA, and re-credentialing on schedule
- If we also bill for you: claims inside CentralReach or the system you use (see our ABA billing page)
What stays with you
- Clinical care, treatment plans and assessments
- RBT supervision and the supervision records BACB asks you to keep
- BACB certification and renewals for your staff
- Hiring, and the supervision ratios on your schedule
Your EHR shows what you billed. Your portal shows what we are doing about it.
Your EHR or practice management system runs your practice, but it does not show you our work. The Medtransic HIPAA Compliant Portal does: every claim and its current stage, the denials we are working, your A/R by age, each payer application and its in-network date, and the documents we still need from you. It is the same record our team works from, and it comes with every client account.
Did that claim get paid?
Every claim with its stage and amount, the denials we're working, and your receivables by age.
Where is my application with Aetna?
Each payer application shows its stage, its history, and the date you can start seeing that payer's patients.
What do you still need from me?
One list of the documents we're waiting on. Upload once, and it reaches every payer that asked for it.
It works in any browser, on your desk or your phone. iPhone and Android apps are coming soon. See the client portal.
BACB credentials, and who can supervise whom
Payers read your roster through these levels. BACB defines them this way.
BCBA: graduate level, practices independently. The BCBA is a graduate-level certification, and BCBAs practice independently. They can supervise RBTs and BCaBAs. Aetna and Evernorth both name the BCBA among those who can supervise ABA cases.
BCBA-D: a designation, not a separate credential. BACB says the BCBA-D is not a separate certification. It grants no privileges beyond the BCBA. For credentialing, a BCBA-D goes through the same process as any BCBA.
BCaBA: undergraduate level, works under a BCBA. The BCaBA is an undergraduate-level certification. A BCaBA may provide services only under BCBA supervision. BCaBAs may supervise RBTs, and they can be supervised by a BCBA or a qualifying licensed psychologist.
RBT: paraprofessional, closely supervised. The RBT is a paraprofessional certification. RBTs work under the direction and close supervision of an RBT Supervisor or RBT Requirements Coordinator. A BCBA can fill either role; a BCaBA can serve as an RBT Supervisor.
BACB's minimum supervision for RBTs. BACB requires RBTs to receive supervision for at least 5% of the hours they spend providing services each calendar month. Supervisors must finish an 8-hour supervision training before they start. Payers can set higher ratios of their own, below.
How we get a BCBA and an ABA agency in network
ABA credentialing has two layers: the agency and the individual BCBA. We run both in order.
State licensure confirmed
We check whether each state where you see clients licenses behavior analysts. BACB notes that some states with new laws may not have an application system yet. We confirm the current status with the state board.
NPIs under the right provider type
NPPES lists separate provider types for Behavior Analyst, Assistant Behavior Analyst and Behavior Technician. Some ABA agencies hold organization NPIs under the Behavior Analyst type. We check that each record matches the person or agency we are enrolling.
Agency contract, then each BCBA
Evernorth contracts with the clinic first, then credentials each certified or licensed provider. Aetna says BCBAs must meet its own credentialing and recredentialing standards. We file the agency contract and each BCBA's application, payer by payer.
RBTs checked payer by payer
Whether a payer or state Medicaid program enrolls RBTs on their own varies. We ask each payer how it wants RBTs listed before we file, rather than guessing.
Tracked to approval in your portal
Each application shows its stage in your client portal, along with any document we still need from you. The portal is included with every client account.
Price and timing: $150 per application, per provider. In our experience it usually takes 60 to 90 days with each payer.
Supervision rules payers publish
Evernorth (Cigna): 1 to 2 hours per 10 hours of treatment
Evernorth's ABA policy calls for case supervision by a BCBA, a licensed behavior analyst, or an independently licensed clinician with documented ABA training. It uses 1 to 2 hours of supervision per 10 hours of direct treatment. When direct treatment is 10 hours a week or less, it expects at least 1 to 2 hours a week.
Evernorth's meaning of direct supervision
Direct case supervision happens during treatment. The BCBA is face to face with the client and with the RBT or BCaBA at the same time.
Aetna: 1 hour per 10 hours, plus an onsite hour each month
Aetna requires ABA to be provided or supervised by a state-licensed behavior analyst or a BCBA. A paraprofessional needs at least 1 hour of face-to-face supervision for every 10 hours of ABA. The supervisor must also be onsite with the child at least 1 hour a month.
Medicaid: EPSDT covers children under 21
Under EPSDT, states must cover any medically necessary Medicaid-coverable service for children under 21, and CMS has said this applies to autism services. How ABA is covered and who can enroll varies by state. CMS's 2026 toolkit adds no new federal requirements.
Medicare: behavior analysts are not on the list
CMS publishes a list of professionals who can bill Medicare for mental health services. Behavior analysts are not on it. We check each payer's rules before filing.
When we are not the right fit
We credential with payers. We do not provide clinical supervision, BACB certification support or exam preparation, and we do not set your supervision ratios.
If your services are paid entirely by families or school districts and you do not want insurance contracts, you do not need payer credentialing.
Questions ABA owners ask about BCBA credentialing
What is the difference between a BCBA, a BCaBA and an RBT?
A BCBA holds a graduate-level certification, practices independently and supervises others. A BCaBA holds an undergraduate-level certification and works only under a BCBA. An RBT is a paraprofessional who works under close supervision.
How much supervision do RBTs need?
BACB requires at least 5% of an RBT's service hours each month. Payers can ask for more. Evernorth's policy uses 1 to 2 hours per 10 hours of direct treatment, and Aetna requires 1 hour per 10 hours plus 1 onsite hour a month.
Do I need a state license as well as BACB certification?
In most states, yes. BACB's table lists 40 states plus DC with behavior analyst licensure laws. Aetna also requires its BCBAs, BCaBAs and paraprofessionals to meet state requirements.
Is a BCBA-D credentialed differently?
No. BACB says the BCBA-D is a designation, not a separate certification, and it grants no privileges beyond the BCBA. Payers credential a BCBA-D the same way.
Does Medicaid cover ABA for children?
Under EPSDT, states must cover medically necessary Medicaid-coverable services for children under 21, and CMS has said this applies to autism services. How ABA is covered, and who can enroll, varies by state.
Does Medicare cover ABA therapy?
Behavior analysts are not on CMS's list of professionals who can bill Medicare for mental health services. That list covers mental health services; we check each payer's own rules before filing.
How much does BCBA credentialing cost, and how long does it take?
Medtransic charges $150 per application, per provider, with no long-term contract. In our experience it usually takes 60 to 90 days with each payer.
