Insurance Paneling for Therapists, Step by Step

Getting paneled means a health plan accepts you into its network, so its members can see you at in-network rates and the plan pays you directly. For a therapist it takes five things: a license at the independent level, an NPI, a complete CAQH profile, an application to each plan, and a signed contract with an effective date. In our experience it usually takes 60 to 90 days with each payer. You can do it yourself; this guide shows how.

The CAQH profile is done once and reused for every plan. Each plan then runs its own review.

Where therapists get stuck

Almost nobody is turned down for the work they do. They stall on paperwork and on picking the wrong door.

Applying to the name on the card

Many plans hand the therapy benefit to a separate behavioral health company. That company runs its own network, so that is where you apply.

A CAQH profile that is not finished or not attested

Most commercial plans read your CAQH profile instead of asking for documents. If it is incomplete, or the 120-day attestation has lapsed, the application waits.

NPI details that do not match the license

Your name, address and specialty on the NPI record should match your license and your application. A mismatch sends the file back.

A closed panel

Some plans stop adding therapists in an area once they have enough. That is a no, not a delay, so ask early whether the panel is open to your license in your county.

Seeing clients before the effective date

Sessions before the contract starts are usually out of network. Do not book in-network clients until you have the date in writing.

How to get paneled, in order

Do these in this order. Each step feeds the next.

Check your license level

Plans panel independently licensed clinicians. Associates and pre-licensed clinicians are usually not paneled with commercial plans; some Medicaid plans differ.

Get your NPI right

Apply for an individual NPI in NPPES if you do not have one. If you practice as a group or LLC, you will usually need an organization NPI as well.

Complete your CAQH profile

Fill in license, education, work history and malpractice details, upload the documents, give each plan access, then attest. Re-attest every 120 days.

Pick plans and apply

Start with the plans your clients actually carry. Apply to the company that runs each plan's behavioral health network, and ask whether the panel is open.

Medicare, if you want it

Since January 1, 2024, LPCs, LMHCs and LMFTs can enroll in Medicare, as LCSWs and psychologists already could. Enrollment is online, separate from commercial plans.

Sign, and get the effective date

Read the contract and fee schedule before you sign. Bill as in network only from the effective date.

Payer notes

Behavioral health networks run separately

The plan on the card and the company that panels therapists are often different. Confirm which one before you apply.

Medicare pays 75%

Medicare pays LCSWs, LPCs, LMHCs and LMFTs 75% of what it pays a clinical psychologist, and you must accept assignment.

Medicaid differs by state and plan

Which licenses Medicaid panels, and whether associates can be billed, depends on the state and the managed care plan.

When to do it yourself

  • If you need one or two plans and have a few spare hours each week to follow up, doing it yourself is reasonable. If you are new to SimplePractice, its free offer covers up to two applications.
  • If you need several plans, run a group, or keep losing track of re-attestation and re-credentialing dates, that is when paying someone pays for itself.

Questions therapists ask about getting paneled

How long does it take to get paneled as a therapist?

In our experience it usually takes 60 to 90 days with each payer. A complete, attested CAQH profile and an open panel keep it at the short end.

Does it cost money to get paneled?

Plans do not charge you to apply, and the CAQH profile is free for clinicians. The cost is your time, or a service: we charge $150 per application, per provider.

Can I get paneled before I am fully licensed?

Usually not with commercial plans. Some Medicaid plans and some commercial plans pay for supervised associates, so check plan by plan.

What does it mean when a panel is closed?

The plan has enough therapists of your license type in your area and is not adding more. A better application will not change it. Ask again later, or ask for an exception if you offer something the network lacks.

Can I see clients while my application is pending?

You can see them as self-pay or out of network. Billing them as in network before your effective date usually is not paid.
