Federal law sets only a floor: under 42 CFR 447.45(d)(1) a state Medicaid agency must accept claims for at least 12 months from the date of service, and states tighten it from there. Medicaid timely filing runs from 95 days in Texas to 180 days in Illinois to six months in California, and each state layers its own balance-billing rules on top of the federal No Surprises Act.
